Full Editorial Writeup
This is a Georgia Medicaid home care agency built entirely around Structured Family Caregiving (SFC), a Medicaid waiver program that pays family members to care for elderly or disabled relatives at home. The agency sits between the state payor and roughly 120 clients and their family caregivers, handling enrollment, compliance, and Medicaid billing. It operates across more than half of Georgia's counties with a remote model: no office, no lease, no vehicles, and no equipment. The core assets are the Medicaid provider enrollment, the active client panel, long-standing hospital and community referral relationships, and a trained staff.
The economics are the headline. On $5M of revenue the business throws off $1.3M of EBITDA, a 26 percent margin, achieved with almost no fixed overhead because caregiving is performed by family members in the home and the agency's job is coordination and billing. Demand is structurally tailwinded: an aging population, a state policy preference for home care over institutional nursing facilities, and a niche with meaningful barriers to entry given the state approval and Medicaid enrollment timeline new entrants must clear.
What makes it notable is the combination of a recession-resistant, government-funded revenue base with an asset-light, relocatable operating model. The seller is retiring and the business is being marketed as turn key at $5M, or 3.85x EBITDA. The obvious thesis is a buyer who can defend the Medicaid relationship, scale the census back toward its peak, expand to full statewide coverage expected in 2027, and bolt on adjacent Medicaid services like personal care and respite onto the existing client base.
