The company acts as the delegated care-management arm of a major managed care organization: its coordinators assess participants, build person-centered service plans, authorize home- and community-based services, and monitor outcomes. Critically, it provides no direct care. There are no aides, no nursing staff, no electronic visit verification burden, and no caregiver payroll, which strips out the thorniest labor and margin problems that plague home-care operators.

Revenue is earned primarily as a fixed per-member-per-month (PMPM) fee that recurs monthly and scales with census and rate rather than hours billed.
