Full Editorial Writeup
This is a Florida-based, debt-free franchisor operating since 2016 at the crossroads of telemedicine, preventative wellness, and anti-aging medicine, with a core focus on Hormone Replacement Therapy and longevity optimization. The hybrid model pairs inventory-free physical franchise clinics with a 50-state telemedicine network, supported by a national diagnostic lab network, board-certified clinicians, and certified U.S. compounding pharmacies. The telemedicine arm is fully relocatable, which gives a buyer real flexibility on cost structure and geography.
The company employs more than 100 people, including general contractors, and reports under 2% turnover among core staff, which signals a stable operating team. It plays in a market the seller pegs at $6.02 billion for 2026, with stated upside from corporate wellness partnerships, multi-unit franchise expansion, proprietary formulations, and AI-driven diagnostics.
The deal is a franchisor sale, not an operating clinic, which matters: a buyer is acquiring royalty streams, the brand, and the recruitment engine rather than a single patient panel. At $6.0M against $583K of cash flow, this is priced at roughly 10.3x, an aggressive multiple that demands the royalty base and franchisee pipeline prove out in diligence.
Why we like it
- The franchisor model generates ongoing royalty and fee income from franchisees rather than one-off transactions, which creates a repeatable cash flow base. With under 2% core staff turnover and a debt-free balance sheet, the operating foundation is cleaner than most listings at this price point.
- The hybrid structure is capital-light on the franchisor side: clinics hold no inventory and the telemedicine arm is fully relocatable across all 50 states. That means a buyer can scale franchise units without the working-capital drag that sinks product-heavy healthcare businesses.
- HRT and longevity medicine sit in a genuine demographic tailwind as an aging, affluent population pays cash for hormone optimization and anti-aging care. The seller cites a $6.02B market for 2026, and cash-pay wellness avoids insurance reimbursement risk entirely.
- A new operator with franchise development or digital-marketing muscle could accelerate unit sales, since the stated growth levers (corporate wellness deals, multi-unit expansion, proprietary formulations) are distribution problems, not product problems. The infrastructure of labs, clinicians, and compounding pharmacies already exists to support more units.
How to improve it
- Audit the franchisee cohort in the first 90 days to separate royalty revenue from one-time franchise sale fees, then build a plan to lift same-unit royalties through better franchisee performance support. Revenue that leans on selling new franchises is far lower quality than a growing base of productive operating units.
- Launch a corporate wellness channel by packaging HRT and longevity programs as an employer benefit, which the seller flags as untapped. B2B contracts would layer in more durable, contract-based revenue on top of individual consumer demand.
- Develop and protect proprietary formulations through the existing compounding pharmacy relationships to create switching costs and margin that competitors cannot easily copy. Defensible IP would justify a higher royalty rate and strengthen the moat against a crowded telehealth field.
- Tighten the regulatory and compliance stack around prescribing HRT across 50 states, since telemedicine prescribing rules vary by state and are actively changing. A clean, documented compliance program both reduces risk and becomes a selling point to prospective franchisees.
- Build a performance-based franchisee recruitment funnel with clear unit economics disclosed in the FDD, so new owners are self-selected for success. Higher-quality franchisees reduce churn and protect the royalty base that this valuation depends on.
- Introduce AI-driven diagnostics and patient monitoring as the seller suggests, differentiating the clinical offering and improving retention of end patients. Better patient outcomes feed directly into franchisee revenue and therefore franchisor royalties.
- Negotiate the asking price down hard given the roughly 10.3x multiple, or restructure with an earnout tied to franchise unit count and royalty growth. The downside protection here lives entirely in the price and the verifiability of the royalty stream.
Diligence notes
- Scrutinize the revenue composition: how much of the $1.09M is recurring royalty income versus one-time franchise sale fees, and how many units are currently open, paying, and in good standing. A 10.3x multiple is only defensible if the royalty base is real, growing, and sticky.
- Review the Franchise Disclosure Document, Item 19 earnings claims, and the full franchise agreement portfolio, including any franchisees that have closed, defaulted, or are in dispute. Franchisee failure rates are the single biggest threat to this cash flow and are often buried.
- Verify state-by-state telemedicine prescribing compliance for HRT and controlled or compounded substances, including the status of the compounding pharmacy relationships and any pending regulatory changes. Anti-aging and hormone prescribing draws regulatory and liability scrutiny that could impair the model overnight.
- Confirm the reason for selling, which the listing simply labels Other, and pin down owner dependence given the 100+ person headcount and contractor mix. With only two weeks of training offered, you need to know exactly what institutional knowledge walks out the door.
- Stress-test the cited $6.02B market figure and the competitive landscape, since cash-pay HRT and longevity telehealth is a crowded and increasingly commoditized space. Understand what actually differentiates this brand versus well-funded national telehealth competitors chasing the same patients.
